hvtiRbootstrap: Bootstrap Model Building for the HVTI CORR Group

Builds models by bootstrap resampling, in the manner of the Cleveland Clinic CORR group's SAS macro library: fit a model on each of many bootstrap replicates, record which variables survive selection, and report how often each appeared. Ports the 'bootreg', 'SUMBOOT' and 'cluster' macros.
